To request CEUs for a meeting or activity
you are organizing online:
- Complete and submit the online
CEU Request Form
(This page works best with Internet
Explorer)
form. The approval will be sent to your email and cc to Tammy Ford-Miller. Tammy will
send you a Sign-In
Sheet with course number and approved CEUs. If
the meeting is longer than one day, one request is needed for each day
separately.

Send your final meeting program to
hailin.zhang@okstate.edu to be posted on this web site after your meeting
is approved.

- Application will be forwarded to the continuing
education committee for approval.
- When approval is received from the continuing education
committee you will be notified that the course has been approved and the hours
awarded to each competency area. You will also be mailed a sign-in sheet.
After the course, return the sign-in sheet to CCA Headquarter.
